Liability WAIVER Form – Learn2Rip Surfing and Ocean Academy
 
This form should be carefully reviewed and signed by the participant (or parent/guardian if the participant is a minor) before engaging in any activities offered by Learn2Rip Surfing and Ocean Academy.

In consideration of being allowed to participate in the activities offered by Learn2Rip Surfing and Ocean Academy, I, the undersigned participant, hereby acknowledge and agree to the following:

1. Assumption of Risk: I understand and acknowledge that surfing and related activities involve inherent risks, including but not limited to the risk of injury from falls, collisions, equipment malfunction, and the actions of other participants and/or instructors.

2. Release of Liability: I voluntarily assume all risks associated with participating in surfing lessons and activities organized by Learn2Rip Surfing and Ocean Academy. I release and forever discharge Learn2Rip Surfing and Ocean Academy, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in these activities, including any negligence on the part of Learn2Rip Surfing and Ocean Academy, its owners, instructors, employees, or agents.

3. Indemnification: I agree to indemnify and hold harmless Learn2Rip Surfing and Ocean Academy, its owners, instructors, employees, and agents from and against any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of or related to my participation in surfing activities.

4. Medical Treatment: In the event of an injury or medical emergency, I authorize Learn2Rip Surfing and Ocean Academy to obtain medical treatment for me and agree to be financially responsible for any costs incurred as a result of such treatment.

5. Photographic Release: I grant Learn2Rip Surfing and Ocean Academy the right to take photographs and/or video recordings of me during my participation in their activities and to use such materials for promotional purposes after receiving my verbal consent prior to the lesson to use these images of me without compensation to me. If you DO NOT want us to take photographs and/or video recordings during the lesson, please check the box below.

6. Acknowledgment of Understanding: I have read this Liability Release Form and fully understand its terms. I acknowledge that I am voluntarily giving up substantial legal rights, including the right to sue Learn2Rip Surfing and Ocean Academy, its owners, instructors, employees, and agents.
